Fingerprint request - No Charge to Cumberland County Residents
Step Two - Petition Packet
1. Completed fingerprint card
2. Name Change Petition for the Court
3. Motion to Waive Publication (two copies – only file if you wish to request to avoid publication)
Step three: Bring everything to the Prothonotary office and pay the filing fee.
Microgrants for name change financial assistance are available through Eastern PA Trans Equity
